Contact Information
The Contact Information question type allows you to ask for a respondents contact details. You can display up to 11 fields, and can choose to exclude fields and change the labels.
The fields that can be displayed are:
- First Name
- Surname
- Address
- Address 2
- Town / City
- State / Province
- ZIP / Postcode
- Country
- Email Address
- Telephone Number
You can choose the label for each of the above, and you can choose which fields are visible. For example, if you wanted all of the above fields to display, except for Address 2 and State / Province, and you wanted Surname to read Last Name, then you would uncheck the visible option for Address 2 and State / Province (meaning they won’t display), and input Last Name in the label option of Surname.

How to add a Contact Information question type
01 When on the ONFORM editor, click the "+" button and select "Question"
A new question block will then load
02 Click on the "Question Type" dropdown and select "Contact Information"
03 Add your question in the "Question" box
This is the question that will be asked, and which respondents can provide contact details in response to.
04 Choose which fields are visible (optional)
Click on the “Display Options” dropdown and uncheck the “visible” toggle for each field you don’t want to display.
05 Change the labels (optional)
You can change the label for each field by typing an alternative in the “Label” box.
06 Adjust settings as necessary by clicking the options tab and choosing the relevant settings.
The available settings are:
- Note (allowing you to provide additional supplementary information)
- Required (the respondent will have to provide an answer to the question)
