File Upload

The file upload question type allows you to collect a file which a respondent uploads, with respondents being able to upload a jpg, jpeg, png, gif, pdf, doc, docx, ppt, pptx, odt, avi, ogg, m4a, mov, mp3, mp4, mpg, wav or wmv.

You can choose to limit which files can be uploaded per question, for example if you are asking a respondent to upload a CV, then you may want to only allow a PDF or DOC/DOCX.

You can download a zip of all the uploaded files on the “Responses Screen”, or you can download uploaded files individually.

You can add up to 20 file upload questions per ONFORM on Solo and Team plans, and on the Enterprise plan you can ask an unlimited number. The total storage space per ONFORM account varies depending on your plan, for the Solo plan it is 5GB, Team 10GB and Enterprise up to Unlimited.

How to add a file upload question

01 When on the ONFORM editor, click the "+" button and select "Question"

A new question block will then load

02 Click on the "Question Type" dropdown and select "File Upload"

03 Add your question in the "Question" box

This is the question that will be asked, and which respondents will provide a file in response to.

04 Adjust settings as necessary by clicking the options tab and choosing the relevant settings

The available settings are:

  1. Allowable file types (choose from jpg, jpeg, png, gif, pdf, doc, docx, ppt, pptx, odt, avi, ogg, m4a, mov, mp3, mp4, mpg, wav or wmv)
  2. Note (allowing you to provide additional supplementary information)
  3. Required (the respondent will have to provide an answer to the question)
